For Your In-Office Appointment
Dr. Warth and his team have taken the following measures to ensure the safety of all patients and staff during office visits:
- All patients, providers, and patient-facing employees are required to wear a face mask. If you do not have a face mask, our office will provide one.
- We have enhanced our already-thorough cleaning and sanitizing procedures throughout the facility.
- We have set up common areas like waiting rooms to allow for social distancing whenever possible.
- Our team also works diligently to minimize the time patients have to spend in the waiting room.
- All patients have their temperature check and are screened for COVID symptoms prior to entering the building.
If you are feeling ill or have recently tested positive for COVID-19, we kindly ask that you contact our office to reschedule your appointment.
When you arrive for your appointment, we ask that you and any guests over the age of 2 wear a mask upon entering the building. Please note that at this time, our office allows only one guest to accompany each patient to their appointments.
Hospital Policies for COVID-19
If you have an upcoming surgery with Dr. Warth, please be aware of the COVID-19 policies. Whenever possible, Dr. Warth performs outpatient procedures so that patients may avoid an overnight hospital stay.
(Note: These policies are subject to change based on the latest scientific data, CDC guidelines and internal guidelines. Please contact the hospital or Dr. Warth’s office if you have any questions about current policies.)
- All patients must complete COVID-19 testing and a medical evaluation prior to surgery.
- Patients are allowed one visitor per day at the hospital, and all visitors must screen negative for COVID-19. Overnight visitors are not allowed.
- Visitors must follow the hospital’s COVID-19 policies, including mandatory mask wearing, frequent hand washing and sanitizing, and social distancing, even while in the patient’s room.
- Visitors must remain in the patient’s room for the duration of the visit, except to get food inside the hospital. If a visitor leaves the hospital for any reason, they are not permitted to return that day.
